Uninsured Services
Non-OHIP covered services are those uninsured services that are provided to patients that are not covered by the Ontario Health Insurance Plan (OHIP) and include but are not limited to the services listed below. Patients will be made aware of these fees prior to the clinic performing the uninsured service.

| Copy of Medical Records | $50.00 + HST |
| Transfer of Medical Records | $30.00 + HST |
| Prescription Renewal by Fax/Phone | $20.00 |
| Attending Physician’s Statement | $160.00 + HST |
**Please note there will be a 2.2% service fee on all invoices paid by credit card (we accept Visa and Mastercard only). Payments made by debit, cash (exact cash only) or cheque will not incur this fee.
